This guide brings together what modern pain science currently understands about pain, and how this understanding can be used to manage pain more effectively, while also respecting the knowledge and insights of the past.
The field of pain science continues to evolve. As our understanding grows, this guide will be updated accordingly.
The goal of this guide is not to provide quick fixes, but to help you understand pain well enough to make informed decisions about movement, recovery, expectations, and daily life. Education does not replace professional care when needed, but it can reduce confusion, fear, unnecessary dependency and even improve the pain experience itself.

Foundations of pain
Before pain can be managed effectively, it first needs to be understood. Modern pain science views pain as a subjective, context-dependent experience rather than a direct reflection of tissue damage. This shift helps explain why pain can persist without injury, why imaging often fails to predict symptoms, and why psychological and social factors matter.
Without a solid foundational understanding of how pain works, it becomes difficult to see why certain steps in this guide are helpful or relevant.
In the Foundations of Pain section, we explore how pain is defined, how it differs from nociception, and how contemporary models have reshaped our understanding of pain.
Load & capacity
Load and capacity are often discussed as primarily physical concepts, yet their influence on pain extends far beyond strength or fitness alone. Capacity is shaped by factors such as sleep, nutrition, stress, psychological state, social environment, and activity levels.
A person can have a strong and capable body and still experience significant pain, just as someone with major physical impairments may experience little or no pain. This highlights that pain is not simply the result of insufficient physical capacity, but of how total load interacts with available capacity across multiple domains.
In this section, we explore how load management and capacity influence pain, and why improving outcomes requires more than the idea that a stronger body automatically leads to less pain.
Beliefs, meaning & psychological modulation
Our psychological state can have a powerful influence on how pain is experienced. Moments of overwhelm, stress, or threat often come with bodily responses such as increased heart rate, sweating, or racing thoughts. These reactions highlight how closely the brain and body interact.
The central nervous system plays a key role in determining how much pain is experienced and when, based on all the information at hand. Thoughts, beliefs, expectations, and meaning shape how the nervous system interprets signals and regulates pain, including processes such as descending inhibitory mechanisms and modulation at the spinal level.
These factors can amplify pain, but they can also reduce it. Mental states such as anxiety and depression often go hand in hand with chronic pain, each reinforcing the other over time.
In this section, we explore how psychological factors influence pain, as well as how cultural beliefs, myths, and taboos can shape our understanding of pain and sometimes hold people back from recovery.
Recovery & physiology
Pain is part of life, and removing pain entirely is not always realistic. For that reason, this section focuses on improving the pain experience rather than promising elimination.
Recovery and physiology play a central role in this process. Factors such as sleep, stress regulation, energy availability, and overall health influence how sensitive the nervous system becomes and how well the body tolerates both physical and psychological demands. These influences become especially important in chronic pain, where recovery capacity is often reduced over time.
In this section, we explore evidence-based strategies that can support recovery and improve pain outcomes, including exercise, manual and non-manual therapies, lifestyle adjustments, and broader recovery optimisation. We also examine the physiological mechanisms that help explain why these approaches can be effective, and how they interact with the pain system.
Clinical applications
Understanding pain becomes most valuable when it can be applied to real-life situations. As someone with a physical therapy degree, I personally would not seek physical therapy again for most pain problems. Not because care has no value, but because once you understand how pain and recovery work, those principles can be applied broadly and independently.
There are, of course, situations where professional care is essential, such as severe injury or neurological events. But for many common pain problems, knowledge about pain, recovery, and adaptation allows people to make informed decisions and apply these principles across different contexts.
In this section, we discuss real-life examples and diagnoses such as patellofemoral pain, chronic low back pain, and rotator cuff-related pain. The goal is to show how the principles discussed throughout this guide can be applied in practice, regardless of the specific label attached to the pain.
Tools to understand pain
Understanding pain is not only about reading or learning concepts. For many people, pain becomes easier to grasp when it can be visualised, experienced, or explained in intuitive ways.
In this section, we introduce tools designed to help translate complex pain concepts into something more tangible and practical. These tools are not treatments in themselves, but educational aids that can support understanding, reflection, and conversation.
They can be useful for people experiencing pain, as well as for clinicians and coaches working with others.

Social & environmental context
Our environment and the people around us play an important role in how we feel, and this includes how we experience pain. Household dynamics, relationships, work demands, and social interactions can all influence mood, stress levels, and perceived safety, which in turn shape the pain experience.
As you may have noticed by now, anything that meaningfully influences us as a human being can also influence how pain is experienced. Pain does not exist in isolation. Factors such as the physical environment we live in, access to green space, financial pressure, and workplace expectations can all affect how the nervous system responds over time.
While many of these influences operate at a broader social or societal level, there are also aspects we can modify, such as the environments we spend time in, the routines we maintain, and the social connections we prioritize.
In this section, we explore how social and environmental factors influence pain, with a focus on practical, everyday contexts such as work, living conditions, and lifestyle. Cultural and belief-based influences are addressed in more detail within the psychological modulation section.
